You have two forms to complete when registering for camp:
1. Enrollment Form
2. FCA Permission Waiver Form
Once you are registered, you will receive a confirmation letter, a statement with your payment information/balance due, a map of UCLA with directions on where to go and a UCLA permission waiver form. This form must be brought to camp! If you do not receive the permission waiver form from UCLA you may click on the link below and print it out for completion.
Please take some time to read the information on our website in Girls/Boys Camp. Many of your questions about camp can be answered in the FAQs and the Transportation page. If you do choose to ride a bus provided by FCA, you must fill out the permission bus waiver form and bring that form with you.
